What will my first appointment entail?


Your first visit will last approximately one hour and will include a comprehensive history and consultation with an applicable

treatment plan. Your physician may review any medical records, diagnostic tests and any recent events that may have contributed to your current condition. Note: The nutritional tools used at the Charleston Wellness Center are not intended as substitutes for any needed medical care or psychotherapy.


Your physician will thoroughly explain your personalized treatment plan and expectations for arriving at a healthier state

of well-being. Communication between the patient and physician is of utmost importance at the Charleston Wellness Center.


When should I arrive?


Please arrive 15 minutes before your scheduled appointment time in order to complete all necessary paperwork.


What should I bring to my first appointment?


Please bring any medical records, diagnostic tests and questions that you would like to be considered in your health care plan.


How much does a treatment plan cost?


The cost of your treatment plan depends on your individual needs. At the Charleston Wellness Center, treatment plans are

customized and vary due to any additional laboratory costs. The initial comprehensive consultation fee is $125. Follow up

office visits range from $85 to $105 depending on time allowed. Payment for services are due at the time of the office visit.


How does continued research benefit patients?

a friendly and encouraging environment. The Charleston Wellness Center’s dedication to on-going research allows it to exceed

customer expectations by providing patients with high-quality care through customized and scientifically proven treatment

programs. Patients are provided with up-to-date evidence-based research for their return to optimal health.




Can I get reimbursed by insurance?

You will receive an HCFA form that you can submit to your insurance company for reimbursement with all the information

needed by the provider filled out. You will need to complete the form with your information. You will be given a template to

use as a guide line to complete the form. Getting reimbursed by insurance depends on your individual health coverage

insurer's current policy and your specific coverage.




For clients wishing to use the services of Charleston Wellness Center as an adjunct to their current medical treatment:


a) The patient acknowledges that the information and techniques presented in this treatment program are designed

for educational purposes only. It is not intended to be a substitute for informed medical advice or care. You should not

use this information to diagnose or treat any health problems or illnesses without consulting your medical specialist or family



b) Further, the patient acknowledges that she is to be under proper medical care and will obtain regular checkups during their program. The patient also agrees to contact their primary healthcare professional for any changes in medications, exercise,

healthcare or diet.
